Applications should be submitted to local agencies of the Social Services Agency under the Ministry or to "DOST" centers or to the "Single Coordination Centers for Applications of Martyr Family Members, War-Injured Military Personnel, and Persons with Disabilities."

They should apply to local agencies of the Social Services Agency under the Ministry and to "DOST" centers.

Admission of individuals to the Center is carried out based on the application of their relatives, legal representatives (guardian, patron), municipalities, non-governmental organizations, as well as other persons with the consent of those persons to the local bodies of the Social Services Agency under the Ministry or to the "DOST" centers.

1. To members of the families of martyrs;
2. To persons with disabilities determined in connection with the war, to children of persons with disabilities determined in connection with the war;
3. To children deprived of parental care;
4. To low-income families;
5. To persons with disabilities.

The assessment and determination of the need for social services is carried out within 10 (ten) working days.

It is regulated by the Decisions of the Cabinet of Ministers of the Republic of Azerbaijan No. 128 dated August 12, 2002, and No. 406 dated December 23, 2021.

Children evacuated from evacuation and resettlement zones, including children who were in their mother's womb on the day of evacuation, as well as children born after April 26, 1986 and whose parents participated in the liquidation of the consequences of the Chernobyl accident or were injured as a result of the Chernobyl accident and were likely to be directly exposed to radioactive radiation, are provided with a sanatorium-resort trip once a year (within the country, including travel expenses, upon referral from medical institutions) by the Social Services Agency and its local agencies, "DOST" centers on a rotating basis.

3. Decision of the TSEK confirming the person's disability (this document is required if it is not possible to obtain it in electronic form from electronic information resources);

The center accepts disabled people aged 15-29 with physical and mild mental and intellectual disabilities.

Minors over the age of 8 who are neglected, homeless, and socially dangerous, and persons over the age of 18 who are in difficult living conditions and have no fixed place of residence, are provided with shelter.

1. To submit an application for the provision of social services;

2. To obtain information on the possibility, procedures, and conditions for the provision of social services, as well as on the forms and types of social services and their own rights and obligations;

3. To participate in the determination and assessment of the need for social services;

4. To choose the social service provider, taking into account the nature of the identified needs;

5. To give consent to the provision of social services or to refuse such services;

6. To demand respect for their honour and dignity and humane treatment by social service providers, as well as to lodge complaints against their actions or inaction through administrative channels and/or judicial proceedings;

7. To be provided, within social service institutions, with conditions that meet sanitary and hygienic standards;

8. To be provided with a separate space for the performance of religious rites, provided that internal disciplinary rules of the social service institution are not violated;

9. To be provided with media resources;

10. To require the confidentiality of personal data;

11. To receive free psychological assistance in accordance with the procedure established by the Law of the Republic of Azerbaijan “On Psychological Assistance.”

1. To provide complete and accurate information for determining the need for social services and making a decision;

2. To immediately inform social service providers of any changes affecting the provision of social services;

3. To comply with the internal disciplinary rules of social service institutions;

4. To treat the honor and dignity of social workers and service providers with respect;

5. To fulfill other obligations stipulated in the Law of the Republic of Azerbaijan “On Social Services.”

4. Relevant certificate concerning persons registered at the individual’s place of residence;

5. The decision of the Medical and Social Expert Commission (TSEK) confirming the individual’s disability (required only if it cannot be obtained electronically from state information systems);

6. For persons deemed legally incapacitated by a court and living in difficult circumstances who are unable to meet their basic жизнененные needs independently: the court decision that has entered into legal force declaring the person legally incapacitated, together with written statements from able-bodied relatives or legal representatives who are legally obliged to care for them but are unable to do so;

7. Two photographs measuring 5.3 × 4 cm.

Able-bodied grandparents, parents, adoptive parents, biological and stepbrothers and sisters, spouses, children, adopted children and grandchildren.

Information is provided regarding the absence of relatives (able-bodied grandparents, parents, adoptive parents, biological and stepbrothers and sisters, spouses, children, adopted children, and grandchildren) who can provide care and assistance.

Legal entities carrying out activities in the field of providing social services to the population, regardless of their type of ownership and organizational and legal form.

In cases where able-bodied relatives or legal representatives who are obliged to care for them have applications stating that they are unable to care for those persons.

Palliative care is the identification, assessment, and provision of assistance for the physical, psychological, moral, and other problems of persons for whom treatment is not possible, as well as their family members.

If a substantiated response is given to a repeat application 3 times within a year and there is no new information to resolve the issue in the repeat application.

The application is reviewed by the Social Services Agency within 10 (ten) working days from the date of submission, and a decision is made regarding the placement of the child in a social service institution.

One of the adult members of the family, or, in the interests of the person (family), the person’s parents, other legal representatives (guardian or custodian), municipalities, non-governmental organizations, as well as other persons with the consent of the individual, may submit an application together with the relevant documents to the Ministry’s “DOST” centers, the local bodies of the Social Services Agency, and the Unified Coordination Centers (UCCs) for applications from members of martyr families, military personnel injured in connection with war, and persons with disabilities.

Lonely elderly people, elderly couples, people with disabilities, people in the terminal (last) stage of the disease who do not live in the same administrative territorial unit in Baku city or in the same administrative territorial district in other cities and regions and who are in need of social services

It is regulated by the Resolution No. 128 of the Cabinet of Ministers of the Republic of Azerbaijan dated August 12, 2002.

Persons and children who have been diagnosed with a disability due to 81-100 percent impairment of body functions, as well as children whose privilege is determined by Resolution No. 128 of the Cabinet of Ministers of the Republic of Azerbaijan dated August 12, 2002.

An application for the provision of home-based (mobile) social services shall, within five working days from the date of receipt by the local bodies of the Social Services Agency (hereinafter – the Agency), DOST Centres (hereinafter – the Centres), or the unified coordination centres handling applications from family members of martyrs, servicemen wounded in connection with war, and persons with disabilities, be referred to a social worker for the assessment and determination of the individual’s (or family’s) need for social services.

Within ten working days from the date the application is submitted to the social worker, the social worker shall carry out the assessment and determination of the individual’s (or family’s) need for social services and prepare a report specifying the form and type of the social service, as well as the place and duration of its provision.

Within five working days from the date the report is submitted by the social worker, the Agency or the Centre shall adopt a decision either to provide social services at the expense of the state budget or to refuse such provision, and this decision shall be delivered (sent) to the applicant within three working days.

From the date a decision to provide social services is adopted, the individual (or family) shall be admitted to social services, and within five working days a social worker shall prepare an individual plan for the provision of social services to the individual (or family).

Graduates of state social service institutions for children, as well as general-purpose boarding and boarding-type special general education schools, who are deprived of parental care and have no place to live.

The relevant category of individuals with creative potential and talent, or their parents, representatives, adoptive parents, guardians or trustees, may apply to the "DOST" centers or local agencies of the Social Services Agency.

Currently, a specialized social service institution for the elderly of the Social Services Agency under the Ministry of Labor and Social Protection of the Population of the Republic of Azerbaijan operates for this category of persons.

The martyr's family member card is issued upon the person's application by local agencies of the Social Services Agency under the Ministry or by "DOST" centers.

1. In cases stipulated in the social service contract regarding the provision of free home (mobile) social services, as well as partially paid and fully paid social services in a guaranteed volume at the expense of the state budget;
2. When a person in need of social services begins to live with his able-bodied relatives or legal representatives in the same administrative territorial unit in Baku city, in the same administrative territorial district in other cities and regions;
3. When lonely elderly people, people with disabilities are placed in state social service institutions;
4. When the rehabilitation of people with disabilities is completed;
5. When a person in need of social services dies.

The services provided by a social worker include the following:              

1. Assisting in preparing meals (including dietetic meals); 
2. Feeding patients who are unable to take care of themselves and washing dishes; 
3. Washing and ironing bedding and clothing; 
4. Tidying up and cleaning the apartment; 
5. Purchasing necessary food, industrial and household goods; 
6. Organizing payment of housing and other utility bills; 
7. Contacting relevant organizations to meet the needs for heating, fuel, and repairs; 
8. Calling a doctor to the house, purchasing medicine from a pharmacy with the doctor's prescription; 
9. Assisting in resolving issues related to the funeral of a citizen who has been taken into social service when he or she dies.

Taking into account the wishes of a minor who has reached the age of 10, his/her parents, relatives, other legal representative (guardian, patron), municipalities, non-governmental organizations, city, district police agencies, local commissions for the protection of children's rights, as well as other persons with the consent of that person, may apply to the Social Services Agency under the Ministry or to the "DOST" centers, taking into account the wishes of the minor, except in cases where taking into account the wishes of the minor is contrary to his/her interests.
